Day Rehabilitation Programme
Our Day Rehabilitation Programme is a team of skilled health professionals including onsite Physiotherapists, Occupational Therapists and Social Workers who work together to support, educate and plan to assist patients with their recovery goals for daily life ensuring older adults have the support required to live as independently as possible following illness or injury.
The programme is designed for those who can reside at home and attend day sessions at Clutha Health First. We provide a complete transport service to and from the programme, a hot meal and all the support needed for patients to succeed in their rehabilitation journey. This may include (where appropriate) input from a Geriatrician, Speech Language Therapist, Dietitian and/or patients GP or Practice Nurse.
Patients are referred to the Day Rehabilitation Programme following discharge from hospital, from your GP, or other health care professional working in either the hospital or the community.
There is no charge for attending the Day Rehabilitation Programme.
