Introduction

Our experienced and highly skilled nursing team provide a wide range of patient-centered care to support people in their everyday lives.

Services are primarily delivered in the comfort of patients’ homes, with clinic appointments also available at Clutha Health First by prior arrangement.

We focus on delivering flexible, high-quality care that meets each person’s individual needs, helping to support health, wellbeing, and independence.

Service Offerings

Our District Nurses provide expert assessment, management support, and education while working with the patients GP or other health professional to deliver the following:

  • ACC Community (Home Based) Nursing 
  • Bowel and Stoma Care
  • Chemotherapy
  • Health Promotion and Education
  • IV Infusion Clinics
  • Meals on Wheels Service
  • Oxygen Therapy
  • Palliative Care
  • Urinary Catheter Care
  • Wound Care, including debridement specialist

 

Referrals and Service Hours

Referrals to the District Nursing service are accepted from a health practitioner or provider. Patients or family members may also self-refer to our service. Electronic referrals are accepted.

Our District Nursing team are available 7 days a week, Monday - Sunday, 8am - 4.30pm, including public holidays by phoning 03 419 0530 or by emailing district.nursing@chf.co.nz 

Our District Nursing service is free to all patients who are eligible for health care in New Zealand hospitals.
