Introduction

Our Day Rehabilitation Programme supports older adults in regaining their independence after illness or injury.

Delivered by a dedicated team of experienced health professionals—including Physiotherapists, Occupational Therapists, and Social Workers—this service provides personalised care tailored to each individual’s recovery goals.

Service Offerings

We work alongside participants to:

  • Improve strength, mobility, and daily functions
  • Build confidence in everyday activities
  • Provide education and practical support for ongoing wellbeing

Our goal is to help people return to daily life with the skills, support, and confidence they need to live as independently as possible.

The programme is designed for people who are living at home and able to attend day sessions at Clutha Health First.

We make participation as easy as possible by providing:

  • Transport to and from each session,
  • A nutritious hot meal,
  • A supportive, well-equipped environment for rehabilitation.

Depending on individual needs, participants may also receive input from a wider team of healthcare professionals, such as a Geriatrician, Speech-Language Therapist, Dietitian, and their GP or Practice Nurse.

Our focus is to ensure each person has the right support in place to achieve the best possible outcomes on their rehabilitation journey.

Access and Service Hours

Entry to our Day Rehabilitation Programme requires a referral from a health practitioner or health provider. Electronic referrals are accepted.

Our Day Rehabilitation Team is available Monday to Wednesday by phoning 03 419 0500.

The Day Rehabilitation Programme is free to all patients who are eligible for health care in New Zealand hospitals.
